Scope

• Leksell Stereotactic System® Service Program consists of maintenance and verification actions to maintain the customer's Leksell Stereotactic System® functionality and performance. Leksell Stereotactic System® Service Program should be continued for a minimum of three (3) years. Devices covered by Leksell Stereotactic System® Service Program
• Leksell® Coordinate Frame G
• Leksell® Multi Purpose Stereotactic Arc, including Arc supports with slides

Service modules included

• Email & Telephone Support, Art No 1503906
• Available during local business hours. Unlimited.
• Site Kit Management, Art No 914379
• Replacement of selected wear and tear parts, once a year.
• Planned Maintenance Local , Art No 1000607 or Planned Maintenance Sweden, Art No 914381
• Verifies functionality and certifies that mechanical accuracy is in accordance with requirements, once a year. Replacement Leksell® Coordinate Frame G provided in exchange to old frame during Planned Maintenance every third year.
• Loaner, Art No 914382
• Loan of a verified Leksell Stereotactic System® during service of the customer unit, if requested once a year.

Preconditions

• Only the devices covered by the Service Program should be shipped to Elekta
• Prior to return shipment to Elekta Leksell Stereotactic System® shall be cleaned and sterilized
• A contamination status certificate shall be filled in by the Hospital and sent with the goods
• The customer system shall be inspected and verified if subscription to the Service Program agreement is requested at a time later than original purchase date.
• Use Planned Maintenance on Demand Local, Art No 1000609 or Planned Maintenance on Demand Sweden, Art No 914385.
• The Loaner system is shipped in a transportation case and the customer shall return their Leksell Stereotactic System® in the transportation case for service to be carried out. The transportation case remains the property of Elekta at all times. Freight costs are included in the price of the program. Loaner is only provided only upon request and max once a year.

Limitations
Damaged parts in need of replacements that not are specified to be exchanged during Planned Maintenance will be charged in accordance to current price list after agreement with the customer.
